All MCA students must have a medical history and physical on file with the school. The deadline to have this document completed & uploaded to your FACTS account is August 1.

Your student's medical history portion must be complete when you arrive for your physical. Physical Form can be found here.
Whether you utilize our physical clinic or you obtain your student's physical from their primary care physician, we will need you to upload both the medical history and the physical to their FACTS account. Instructions for upload here.
Volunteer as a Parent
